Back Pain can be a symptom of many diseases, illness and conditions. The main cause of the Pain can be a problem with the back itself or by a problem in another part of the body. Back Pain affects females from a very early age and at every stage of their Lives viz. Puberty, Periods, Productive Age, Pregnancy, Post Partum, Pre-Menopausal Age, Post-Menopausal Age etc.

Hormones can cause havoc to a Female physically, mentally, emotionally and their interaction may have a deep-rooted role in altering & alleviating the whole mechanism of pain & the painful experience.

Puberty & Female Back Pain:

Puberty for girl is like floating down a broadening river into an open sea. Going through Puberty as a young girl is so confusing. The monster invades your body, changes and makes things grow and no one tells you what’s going on. Puberty is Growing Pain; it starts with Immature and ends with Mature.

For Females there seemed to be an increase in Low Back Pain reporting around the age of Puberty. Two possibilities for this are first, the growth spurt initiated during puberty could be the starting point of Back Pain & Back related problems and second, hormone induced changes at puberty might have an effect on attitudes to or perception of Pain.

Periods & Multifaceted Back Pain:

Menarche is the start of periods. It will occur when all the parts that make up a girl’s reproductive system are mature and working together. A girl’s periods generally begin between the ages of 12 and 14 years, but this can vary from 8 to 16 years. Menstruation is a major stage in a Girl’s Puberty and is one of several physical signs that a Girl is becoming a Woman.

Periods are ridiculous! Girl’s shouldn’t be punished for not getting Pregnant.

With the onset of Menstruation & Ovulation in Girl’s Life, a number of clinical Pain conditions show variation in symptom severity across the Menstrual Cycle.

  • Pre-Menstrual Syndrome (PMS) symptoms especially Mild to Moderate Back Pain occur within the week before period and stop once periods start. For some females, Severe Lower Back Pain is a frequent symptom. This may be related to increased Inflammation during Menstruation.
  • PMDD (Pre-Menstrual Dysphoric Disorder) is a more severe condition& experienced by many a female than PMS. It’s characterized by severe PMS symptoms that can interfere with one’s Daily Life, including Work, School, and Personal Relationships. Like PMS, an increase in Inflammation can be a cause of Severe Lower Back Pain in PMDD. However, it could also be a side effect of other PMDD symptoms, such as Diarrhoea, Vomiting & Pelvic Pressure.
  • Pain of Dysmenorrhoea –is a condition characterized by Painful Period Cramps with Severe Low Back Pain, Thigh Pain, Leg Pain etc. Period cramps from Dysmenorrhea can radiate throughout the entire Lower and Upper Back.The Pain can be anything like – Spasmodic, Shooting, Piercing, Stabbing etc.
  • While some Lower Back Pain is normal during Periods, Severe & Constant Lower Back Pain can indicate a more serious issue, such as Endometriosis which is a condition characterized by the displacement of Uterine tissue outside of the Uterus. This tissue most commonly gravitates toward other areas of the pelvis causing Severe Period Pain including the Lower Back Pain, Chronic Pelvic Pain especially during and after sex, Pelvic Pain outside of menstruation, Heavy & Painful Periods that may be longer in length etc.Back Pain from Endometriosis might feel different than Back Pain from PMS, PMDD, or Dysmenorrhea & is a Serious Condition which needs formal diagnosis & Treatment.

Productive Age & Back Pain:

The combination of demanding targets with long working hours with the vigorous demands of household chores takes a toll on Woman’s Health. Women generally have a habit of ignoring their own health or their health issues. So, when a symptom like Back Pain is ignored for a long time then it is converted to Chronic Back Pain.

  • Also, it is a fact that women of the age group 35 years and above are subject to the early degenerative changes. These degenerative changes lead to pain as the bones get weak and brittle. The back pain could be because of the Strained Muscles or the Weak Bones – Osteoporosis.
  • Use of Oral Contraception is linked to Low Back Pain – The longer a woman used oral contraception, the more likely she was to have Chronic Low Back Pain. Non-pregnant women taking oral contraception have increased levels of Hormone Relaxin, which compromises the support of the joints. Loose joints are at risk for injury.

Pregnancy & Back Pain:

It is very common to get Back Ache or Back Pain during Pregnancy, especially in the early stages. The Ligaments naturally become softer and stretch to prepare for labour. This can put a strain on the joints of the Lower Back & Pelvis, which can cause Severe Cutting Back Pain.

  • During Pregnancy, the cyclical fluctuations in hormones cease and instead a steady increase in the levels of both MProgesterone & Oestrogen is seen towards term which fall rapidly after delivery. Prolactin & Relaxin Hormones are secreted and many Clinical Pain Conditions improve during Pregnancy including Arthritis, Migraine and often Pelvic Pain However, Pregnancy itself can be associated with the development of Pain especially Mechanical Back Pain and Symphysis Pubis Dysfunction (SPD).

Post Partum & Back Issues:

Caesarean Sections, Painless Deliveries are mostly performed these days and to reduce pain during Labour and Delivery an increased Epidural and Spinal Anaesthesia are performed. The Needle is usually placed into the L4 L5 Disc and anaesthetic medicine is sent into it and while the needle is retracted back from the disc, the disc bulges out and the first stage disc problem eventually starts up. The handling of the Child, Breast feeding, Faulty diets and Life styles further shifts the so called first stage Disc Problem – Disc bulges to the next level of Disc Diseases like Disc Protrusion, Disc Extrusion & the Sequestred Disc.

Pre-menopausal Age & Back Pain

Menopause is the natural aging process & is the end of female reproduction. There are different stages within Menopause that are important to recognize and understand. Menopause occurs when menstruation stops completely.

Premenopause, is defined as “Before Menopause” a stage of Reproductive years having Periods (Regular or Irregular), slight or no hormonal changes and no symptoms and no noticeable changes in the body.

Perimenopause,, on the other hand is defined as “Around or Near Menopause” or the ‘Menopause Transitional Phase’, a phase where there is a decline of Ovarian Functions, starts experiencing symptoms of Menopause, and ends a year after last menstrual period. Symptoms may range from Mild to Moderate Spine and Joint Pain, Hot Flushes, Night Sweats, Chronic Tiredness, Anxiety, Mood Swings, and Sleep Disorders etc.

Menopause, the Ultimate Climacteric can cause an array of symptoms and Chronic Pain diagnoses such as Fibromyalgia, Migraine, Arthritis and Severe Low Back Pain in many way –

  • Weight Gain during Menopause increases pressure on the Lower Back & increases the severity of back pain.
  • Stress and Anxiety, common psychological symptoms of menopause, increase muscle tension throughout the body, including the Lower Back.
  • Decreased Oestrogen is associated with increased Disc Degeneration in the Lumbar Spine, which contributes to Lower Back Pain.
  • Decreased Oestrogen is also associated with Elevated Cortisol Levels. Both Blood Pressure and Blood Sugar rise with cortisol, which contributes to muscle tension and spasms throughout the body, weakness, & fatigue.
  • Decreased Testosterone, decreases the muscle strength and tonicity. Weaker back and abdominal muscles contribute to Lower Back Pain.

Post-Menopausal Age & Back Pain

These are the years “After Menopause”. During this stage, Menopausal symptoms, such as hot flashes, can ease for many women. But, as a result of a lower level of Oestrogen, postmenopausal women are at increased risk for a number of health conditions, such as Upper Back Pain, Osteoporosis, and Heart Diseases etc.

Osteoporosis is is a condition that causes the thinning of the Bones. This change in bone density is due to the loss of Oestrogen. Osteoporosis makes the post-menopausal women susceptible for Bone Fractures particularly in the Hips, Spine, and Wrists.

HRT (Hormone Replacement Therapy) which are usually prescribed in Post-Menopausal age group carries many risks of Breast and Uterine Cancer, Stroke, Heart Disease, Deep Vein Thrombosis & Gall Stones etc.

Women’s Health & Ayurveda:

A Woman is considered to be ‘Shakthi Swaroopa’ & Women’s Health encompasses many different issues during the different stages of her life. Every woman goes through a sequence of changes during her Lifetime viz, Puberty, Periods, Productive Age, Pregnancy, Post Partum, Pre-Menopausal Age, Post-Menopausal Age etc.

However, Ayurveda considers Hormones as “Shukra Dhatwagni” and is carried by the Blood (Raktha) and is a component of Pitta Dosha.

Maintaining a Healthy Menstrual Cycle and the balance of the Hormones will have a deep rooted impact on Woman’s Body, Health, Vitality, & General Wellbeing. Understanding one’s Personality or the Basic Body Constitution (Prakrithi) is important in Ayurveda. The predominance of ‘Dosha’ – Vata, Pitta & Kapha will help to understand how body works and changes can thus be made to help the body come back to normalcy.
